MEMBERSHIP FORM - printable - one form per child, either fillable or printable format (if you haven't filled out 2024 yet)
PARENT HANDBOOK - for your reference
ANAPHYLAXIS FORM - only if applicable to your child
DIABETES MANAGEMENT FORM - only if applicable to your child
MEDICATION AUTHORIZATION - only if applicable to your child and mediation will be taken during the day at camp
